
高血压,是这两年患者增加最多的疾病之一。根据估计,我国目前有5亿成年高血压患者。无论是健康人群还是高血压患者,一天24小时内的血压值都会随着昼夜节律有所波动,比如高血压患者常常在清晨血压最高。简单的血压升高,会给我们的身体带来不少的问题,心血管疾病、肾病、痴呆等的风险都会因高血压而升高。
降压药是控制血压的重要方式。对于大多数人而言,降压药的服用时间没有太大影响,只要每天按时服用就行。然而实际上,药物的效果会随着在体内的代谢而在一定时间后逐渐减弱。由此不难得出,服药时间可能会影响血压控制情况。
近日,西班牙维戈大学的研究团队,在对近2万人进行了6.3年的临床试验后发现,相比早起后服用降压药,在睡前服药可以降低45%的主要心血管事件风险,心血管死亡、出血性中风、心衰的风险分别降低56%、61%和42%。也就是说,通过改变服药时间,我们最多能在原有基础上降低一半左右的心血管风险。该研究发表在《欧洲心脏杂志》上。
目前诊断高血压的标准指的都是白天安静时的血压值。而人在一天中的血压是不同的,正常人在白天血压较高,而入睡后会降低10%~20%。这样,描记下来的24小时血压曲线形如一把勺子,被称为“勺型血压”。
与之对应,有些人的血压,在睡眠期间下降不足10%,被称为“非勺型血压”,其中甚至有人夜间血压反倒升高,成为“反勺型血压”。相比于正常的勺型血压,这样的非正常血压波动,与多种心血管风险升高有关。而动态血压监测中的睡眠血压平均值,也是一个比日间血压或24小时血压平均值更为敏感的心血管预后指标。
血压有昼夜波动,那降压药的治疗效果会不会也与服药时间有关呢?确实,先前的研究发现,在睡前服用降压药,能让一天中的血压波动更为接近正常的勺型。这次,研究人员更进一步,探究了睡前服用降压药对心血管风险的影响。
研究人员们招募了19084名正常作息的成年血压患者,他们平均年龄60.5岁,10614人为男性。参与者们被随机分配在晚上睡觉前或早上起床后根据医生建议服用降压药,并接受至少每年一次的随访,化验血尿,测量48小时动态血压。
他们服用的药物包括血管紧张素Ⅱ受体拮抗剂(ARB)、血管紧张素转化酶抑制剂(ACEI)、钙拮抗剂(CCB)、β受体阻滞剂和利尿剂中的一种或几种,并会在每次随访时根据血压控制情况进行调整。使用最多的还是ARB和ACEI这两种RAAS相关的降压药。
经过中位6.3年的随访,一共出现了1752例主要心血管结局,包括心肌梗死、冠状动脉血管重建术、心力衰竭、缺血性卒中、出血性卒中和心血管死亡。

各种心血管指标上,睡前服药的患者,睡眠期间的血压显著低于早起服药的患者,而他们白天时的血压相近,这也使得高心血管风险的非勺型血压在早起服药的患者中更多。此外,睡前服药的患者还有着更低的血LDL和肌酐水平,更高的血HDL水平。
最为重要的心血管风险上,在排除了年龄、性别、病史等诸多因素的影响后,研究人员发现,相比早起后服药,睡前服用降压药可以降低45%出现主要心血管结局的风险。其中,心血管死亡、出血性中风、心衰、外周动脉疾病的风险分别降低了56%、61%、42%、48%。而不良反应和依从性上,两组患者并没有明显差距。
研究人员表示,目前的高血压治疗指南没有提到或推荐任何首选的治疗时间。基于降低早晨血压水平这一误导目标,医生们最常建议早上服药。这项研究的结果表明,睡前服用降压药的病人比醒来时血压控制得更好,最重要的是,他们死于心脏和血管疾病的风险大大降低。
