  偏头痛(Migraine)是一种常见的慢性疾病,主要症状包括出现反复轻度或重度头痛,通常伴有各种自主神经系统症状。偏头痛的英文“Migraine”一词源于希腊语“hemicrania”,意为“头部一侧的疼痛”,其中,“hemi-”,意为“一半”,“kranion”,意为“颅骨”。也就是说,这里的“偏”指的是单侧。

  偏头痛会伴有搏动,持续2-72小时。严重者还会恶心、呕吐、对光线更加敏感、对声音更加敏感,且肢体活动会加重疼痛的感觉。大多偏头痛患者能够感受到短暂的视觉、感觉、语言或肢体障碍。

  我们为什么会患有偏头痛呢?

  偏头痛主要患者为女性,激素水平的不正常波动便是其中一个重要原因。并且,偏头痛的病因被认为是环境因素和遗传因素共同引起的。在青春期之前,患有偏头痛的男童比女童要稍多一些,然而青春期一过,受到偏头痛影响的女性则比男性要多出两至三倍。女性只有才通常在怀孕期间,偏头痛的影响会减弱。

  中枢神经系统结构(如脑干和间脑)起着致痛的主导作用,还有一些理论则支持周边激活理论(如通过围绕在头部和颈部血管周围的感觉神经激活头痛导致)。可能触发头痛的血管包括:硬膜动脉、软膜动脉和颅外动脉,如头皮动脉等。颅外动脉的血管舒张尤其被认为是重要致病原因。也就是说,我们现在主要认为,偏头痛是神经血管紊乱所导致的,但其致病原理目前尚不得而知。这也是为什么,我们如今还无法根治偏头痛,只能通过一些方法来减缓。

  大多数偏头痛患者,使用急性药物来减少急性偏头痛的发作,也就是常说的止痛药,包括布洛芬和醋氨酚等等。

  在临床实践中,医生有时也使用非药物性的预防性治疗手段,比如有氧运动。

  为了弄清有氧运动对治疗偏头痛是否真的有效,安特卫普大学的研究人员对有关有氧运动对偏头痛的天数、持续时间和疼痛强度的影响的已有文献进行了回顾,将具有相同的偏头痛诊断和结果测量单位的文章的结果进行汇总,最终总共累计有176名参与者。

  结果发现,有氧运动将偏头痛的患病天数减少了22%至78%。当进行10-12周的有氧运动时,能显著影响偏头痛的天数。有氧运动组的偏头痛天数平均减少了0.6 ± 0.3天/月。

  在临床上,这一发现的相关性不高,因此需要有更多的研究来探讨有氧运动对偏头痛患者的影响。有趣的是,有研究表明,高强度间歇训练具有更强的治疗效果,但是在统计学上并不显著。

  基于三项研究的分析结果表明,有氧运动后疼痛强度下降了20%-54%,并且持续时间减少了20%-27%。而与对照组相比,在锻炼组患者中使用急性药物则没有显著变化。

  研究人员还发现,作为预防性药物的托吡酯和三环类抗抑郁药,在减少每月偏头痛天数方面的效果与有氧运动相似。然而,药物治疗常伴随有副作用,会造成体重变化、记忆力减退、疲劳等反应,因而有氧运动似乎是一种很有效的替代方案。就像生活经验告诉我们的那样,有氧运动没有引起任何副作用。

  但令人失望的是,参与实验的被试很多人都中途退出了。退出最重要原因是,他们没有时间参加每周三次受监督的运动训练课程。而那些坚持下来的参与者,都获得了一定程度的安慰。

  有氧运动为什么有效?

  主要原因在于:有氧运动可以通过调节大脑中的疼痛处理过程,从而减轻痛感。除了偏头痛,运动已被用于治疗各种急性和慢性的肌肉骨骼疼痛病症,如纤维肌痛、慢性疲劳综合症、腰痛等等。

  虽然减缓原因不够详细,但是人类对于运动治疗的历史非常久远,可见两者的相关性在经验知识时代就已经被发觉。

  由三国时代华佗发明的五禽戏,等运动一直以来被用来防治疾病;古代印度有各种瑜伽的体位练习和运动方法;古罗马也有医生用运动治疗多种疾病;古希腊医学家阿莱特乌斯(Aretaeus)等人对运动维持健康、增强体力、治疗疾病给予很高的评价。

  同时,现代医学中越来越多的证据告诉我们:体力活动和适量的运动可以减缓疼痛,改善身体机能,并对心理机能和生活质量产生不同的影响。

  因此,与其在药物治疗上投入更多的金钱和时间,不如调整好作息,做一个健康生活的都市青年。
