  2月11日,世界卫生组织宣布,新冠病毒 2019-nCoV 引发的疾病正式命名为 COVID-19。目前,这项疾病已经造成超4万人感染,死亡人数超过1000人。在不少有关 NCP 的讨论中,人们将其与美国的流感季进行了对比。根据美国疾病控制与预防中心发布的数据,截至2月1日,此次流感季中预计有2200万~3100万人受感染,因流感死亡的患者人数在1.2万~3万人之间。从数字上来看,美国的流感疫情似乎更加严重。然而事实是怎样的呢?两种疾病之间又存在什么差异呢?

  流感虽然全称流行性感冒,但是它却与普通感冒关系不大。流感病毒和 2019-nCoV 都属于可引起呼吸道疾病的病毒,相比普通感冒,它的致病源不同,因此也就具有更强的传染性和更高的致死率。与 2019-nCoV 相比,两者最大的差异就是人类的认知程度。尽管流感的发病率和死亡人数也很高,经过数十年的研究,科学家已经掌握了流感的爆发规律,对流感的治疗手段、发病症状以及可能的流感暴发季节都有着充足的了解。相比之下,我们对 2019-nCoV 的了解却很少,因为它是一种全新的病毒。两种病毒在病毒传播、临床症状与治疗等方面也存在诸多差异。我们还处于对 2019-nCoV 的探索阶段,我们对它及其威胁的认识也会随着研究深入而改变。

  2019-nCoV具有强大的传染能力,“基本传染数”(R_0)被科学家用来评估病毒传播的难易程度。通俗地说,就是评估一名感染了某种传染病的患者,平均能够传染给多少人。R_0值越高,这种传染病的传染能力越强。根据《纽约时报》的报道,流感病毒的 R_0 值约为1.3。而对于 2019-nCoV 的 R_0,研究人员仍在努力确定。随着科学家的研究,它的 R_0 估计值从1月末的2.2提升到了目前的3.77。这意味着 2019-nCoV 的传染能力远超此前的预期。每种疾病的 R_0 值并不固定,因地区而异,具体取决于人们相互接触的频率,以及防疫措施的强度。

  病毒潜伏期的长短也影响疾病传染能力,流感病毒的潜伏期多在1~4天,2019-nCoV 潜伏期的中位数为3天,但更加复杂,不同患者间存在显著差异:患者最快在当天发病,而一名患者自称经过24天的潜伏期才显现出症状。

  流感病毒的人与人传染主要通过直接传播气溶胶传播和接触传播三种方式实现,进入人体后附着在呼吸道内细胞上。而目前已经可以确定的 2019-nCoV 的传播途径包括直接传播和接触传播,而它能否进行气溶胶传播和粪口传播,目前证据仍不充分。2019-nCoV 与 SARS 一样,通过 ACE2 受体进入细胞,继而感染呼吸道。

  流感的典型症状包括发烧、咳嗽、咽喉痛、肌肉酸痛、头痛、流涕或鼻塞、浑身乏力以及偶发的呕吐或腹泻。流感的症状通常是突然发作的,大多数流感病毒的感染者会在两个星期内康复。NCP 最普遍的症状是发热和咳嗽,腹泻、呕吐等消化系统的症状极为罕见。美国此次流感疫情的致死率不到0.1%,大约1%感染者的症状程度需要住院。需要指出的是,大量流感患者可能不会前往医院,或是没有进行流感病毒检测,一些人可能因流感导致的并发症(如肺炎、心血管疾病等)死亡,所以美国流感感染者、死亡人数等数据不是通过直接统计得出,而是基于美国 CDC 建立的动态模型的评估结果。综合现有研究的判断,2019-nCoV 感染的致死率约在1%~3%之间,远高于流感。NCP 的重症患者比例为14%,比流感更高,严重程度远超流感。不过,随着更多轻症患者得到确诊,COVID-19 的死亡率可能会下降。

美国 CDC 建立的动态模型的评估结果

  由于对流感的研究已经比较深入,我们可以通过注射疫苗来预防60%的季节性流感,同时人们也找到了较为有效的抗病毒药物。但 2019-nCoV 是一种新发现的病毒,针对NCP的疫苗研发工作还仍正在进行中,距离最终问世还需较长时间。针对它的特效药也仍在寻找中,距离药物通过临床测试上市,同样需要一段时间。

  季节性流感每年都会出现,与流感大流行事件、新型流感病毒的暴发在严重程度上有着显著区别。例如,美国 CDC 的资料显示,2009年爆发的猪流感就是一次流感大流行事件,估计共造成了全球约15.1万~57.5万人的死亡。而目前,NCP 疫情还并未被评估为全球大流行事件。其原因在于,目前大多数病例只发生在中国,并未在全球其他国家或地区呈现暴发态势。此前 WHO 宣布 NCP 疫情是“国际关注的突发公共卫生事件”。这一声明主要是担心该病毒可能会传播到卫生系统较弱的国家。
